Khotang is an eastern hill district where deep river valleys, forested ridges and Kirati cultural heritage meet major pilgrimage sites. Halesi Mahadev is the best-known attraction, drawing Hindu, Buddhist and Kirat visitors to a landscape of caves and sacred rock formations. The district's high ridges also connect with the wider Mundhum cultural trail system.

With an area of 1,591 sq. km and a 2021 population of 175,298, Khotang sits within the extraordinary geographic diversity of Koshi Province. The density figure of about 110 people per sq. km is a useful administrative measure, but it does not fully describe the lived geography: settlements, forests, river valleys and ridgelines can change sharply across short distances.

Halesi Mahadev, globally famous among Buddhists as the Maratika Cave, is a highly sacred tri-religious cave temple located next to Mahadevasthan village in Khotang, at an altitude of approximately 1,300 meters. The cave temple is steeped in ancient mythology, being revered equally by Hindus, Buddhists, and Kirat followers. For Hindus, it is the 'Pashupatinath of the East,' where Lord Shiva hid from the demon Bhasmasur; for Buddhists, it is the cave where Guru Padmasambhava attained the siddhi of long life; and for Kiratis, it is their ancestral dwelling.
